The one-time fee includes an online physician consultation, medical marijuana recommendation and requisite follow-ups. Medical Marijuana recommendations will be provided to you on the same day of your consultation. Medical Marijuana recommendations are valid in Missouri for one year!
You are responsible for submitting your approved medical marijuana recommendation to the Missouri Department of Health & Senior Services Missouri Department of Health & Senior Services online registry within 30 days of receiving it. If you do not submit within 30 days, your recommendation will no longer be valid. You will need to be recertified by one of our physicians for an additional fee. Click the blue 'PATIENT/CAREGIVER APPLICATION REGISTRATION' button and follow the registration instructions.
During your renewal consultation, the physician will review your medical marijuana treatment while providing additional information, advice, updates and answering questions you may have. All requisite follow-ups are included. Renewal recommendations are valid for one year.
You are responsible for submitting your medical marijuana recommendation renewal to the Missouri Department of Health & Senior Services within 30 days of receiving. If you do not submit within 30 days, your recommendation will no longer be valid. You will need to be recertified by one of our physicians for an additional fee. $25 | The standard and required application fee for patients
$25 | Optional, only if you need to add a caregiver to your account.
Application fees must be paid by Visa, MasterCard, or Discover credit or debit card. An additional credit card processing fee will be added to the transaction. These application and processing fees are non-refundable. The payment will not be refunded for applications not approved.

Given our volunteering, philanthropic and charitable efforts, Green Team Doctors are highly respected throughout the community.
The founders are early pioneers of the medical cannabis industry having been involved in dispensary operations in Michigan and technology innovations in California. Unfortunately, both lost their fathers' to the opioid crisis and became passionate about exposing the masses to medical cannabis in hopes of potentially saving lives.

Dr. Bernard Miller, MD is a top-level clinical and translational physician-scientist entrepreneur with over 20 years of professional experience. Dr. Bernard trained & conducted research at the University of Michigan Medical School, Columbia University College of Physicians and Surgeons, Washington University in Saint Louis School of Medicine, and the Intramural Research Program of the National Institute of Diabetes/Digestive/Kidney Diseases and the National Institutes of Health. He is a compassionate human being who cares about delivering effective solutions to his patients for their health problems. His main goal is to provide options to patients that have not found relief or who are at risk for complications when using traditional medical therapies.

Dr. Juilanne Moy graduated from Missouri University Heritage College of Osteopathic Medicine in Athens, OH and continued her clinical training in southern Missouri in the specialty of family medicine. Currently she practices urgent care medicine in the greater Columbus region. She was one of the first CTR (cannabis) physicians in Missouri.

A recent report by the Cincinnati Enquirer states 3/10 (3.5 million!) Ohioans qualify for medical marijuana!
To qualify you must:
- Possess a qualifying symptom or condition as set forth by Ohio Law
- Present a recommending physician with an unexpired United States passport, United States passport card, state-issued drivers license; or other state-issued identification issued by the Ohio bureau of motor vehicles.
- Be at least 18 years of age
- Gain written recommendation from a physician who has obtained a certificate to recommend from the State Medical Board of Ohio.
All OGT physicians are certified by the State of Ohio
Ohio currently has 21 approved symptoms & conditions.
Under Ohio law, all of the following are qualifying medical conditions: Chronic traumatic encephalopathy, AIDS, amyotrophic lateral sclerosis, Alzheimer's disease, cancer, Crohn's disease, epilepsy or another seizure disorder, fibromyalgia, glaucoma, hepatitis C, inflammatory bowel disease, multiple sclerosis, pain that is either chronic and severe or intractable, Parkinson's disease, positive status for HIV, post-traumatic stress disorder, sickle cell anemia, spinal cord disease or injury, Tourette's syndrome, traumatic brain injury, and ulcerative colitis.
Additional symptoms are subject to change and may be added to this list at the discretion of the State of Ohio Medical Marijuana Control Program.
